Wildermire, who is 74 years old, was admitted to The Bristol at Tampa Rehabilitation & Nursing Center on December 23, 2020, from Florida Hospital, formerly known as Advent Health in Tampa, FL. Wildermire was diagnosed with chronic pain and severe swelling of the left lower extremity. Wildermire was greeted by our administrative team and presented with a welcome bag by our Concierge Services Director. Wildermire shared with the team her pain was aggravated by any slight movement and needed immediate physical therapy. The team did not hesitate and quickly worked on her care plan. Ms. Wildermire was evaluated by several team members and was ready for her journey to begin.

Week 1-2: Physical therapist team member Tessa worked on Ms. Hamilton’s evaluation and plan of treatment for both short-term and long-term goals. One of Ms. Hamilton’s goals was to get back home, as soon as she could, to be with her family. Ms. Hamilton shared she lived at home with her niece and could not wait to be home with her again. During a weekly concierge interview, Ms. Wildermire shared she was pleased with the services provided by the entire team. She stated, “Everything is going well thus far, and the staff here are wonderful. My room is cleaned daily by housekeeping and my compliments to your dietary department. The food is good here. Thank you all for everything.”

By week 3-4, Ms. Hamilton successfully met all short-term and long-term goals during her recovery at The Bristol. Her functional performances had improved remarkably because of instructions in compensations, modifications, and adaptations to treatment by the occupational therapist, Thomas, and physical therapist, Tessa. Ms. Hamilton improved all ADLs, or activities of daily living, safely and independently.

By week 5, Wildermire Hamilton completed both physical and occupational therapy and received her discharge date of January 19, 2021, to return home with recommendations of a home exercise program, Home Health Services, and an assistive device for safe functional mobility by her Attending physician, social worker, risk manager, and care plan team.
